How can you build a city on top of a city?

Do you get lots of dirt and cover it over? (Some cities, like Seattle, have the old city underneath them, which you can tour. How does that happen so that it is still accessible?)

You would have to tunnel out for the underground city. How could you walk or drive if you didn't do this?

They do exactly that. In Sumeria they filled in the first story with trash and built another floor above. There was often no garbage pickup.

Well Pompeii for example. It fell victim to a volcanic eruption. A new city was built over the charred ashes. They were able to excavate artifacts, and structures preserved in dried molten rock.
